PADI Open Water Diver certificate
A student diver must present the PADI Universal Referral Form for Open Water Diver Certification to a dive instructor. It states that the student diver has completed initial training with another dive training organization. This form instructs the instructor to complete the student’s open water dives and sign a statement. The form should then be sent to the original instructor who should certify it.
The Universal referral form can be used to get certification. But, PADI demands that you complete the course within 12 month of your last training. This usually takes two to three days. You will be required to be able dive with your buddy at a maximum depth to 60 feet/18 meters.

Statement for medical purposes
You will need to fill out the Medical Statement for Universal Reference Form if you want to become a scuba diving instructor. This form is essential to make sure that you are as healthy and fit as possible before you start diving. This document is designed to protect you underwater. Be sure to answer all questions honestly.
First, ensure that your doctor signs the form. This is mandatory when you begin your training as an scuba diver. You will need a new medical release if you have any medical conditions. Finally, it is a good idea to always have a photocopy on hand of your certificate card.
Logbook
A medical declaration is required before a diver may dive open water. This is usually done before certification courses begin. The medical release must be signed by a physician. Changing medical conditions require a new medical release. A medical release is available through PADI. The medical release is equivalent to the PADI Release of Liability/Assumption of Risk/Non-agency Acknowledgement Form - General Training or the EU version.
The universal referral is different from that used in a diving logbook. It will list the open water dives and knowledge-development sections the student has completed. This document also includes a statement signed by the instructor verifying that the diver has completed the required dives. The student must then take this form to the instructor, who will certify him/her as a certified diver.

Requirements for course completion
The Universal Referral Program involves most well-known certification agencies in scuba diving. The Universal Referral Program allows divers to take the theory and pool portion of their open water course at their local dive center, but then complete the open water dives with another agency. This can be accomplished with the assistance of a dive shop that participates in a universal referral program, such as SSI/NAUI/NASDSS/YMCA or any other organization.
PADI Open Water Diver certification requires that students pass both the Knowledge Development section and Open Water Dives section. Additionally, the student diver must provide a medical release signed by his or her physician before diving. A photo must be submitted by the student diver to accompany his/her certification card. Digital images are acceptable.
